Welcome to our Patient Resources Center. Here you can access patient forms online, access skin assessment and aftercare pamphlets, and learn about the insurances we accept. 

We appreciate the trust you have placed in us and want you to know we are committed to doing everything we can to make your experience as comfortable and pleasant as possible.

Insurance and Payment

Dr. Zanelli participates with Medicare and some PPO insurance plans (HealthNet, Blue Cross, Cigna) for medically necessary treatments and evaluations.  For patients with plans with which we do not participate, payment is made at the time of service and patients may submit the medical charges to their insurance plans for out of network benefits reimbursement.

Cancellation

Cancellation policy:  We do our best to confirm all appointments ahead of time.  We request the courtesy of a cancellation notification if you will not be coming for your appointment.  In order to avoid a first appointment cancellation charge of $60, or $100 for second and subsequent cancellations, patients must keep the appointment as scheduled, or cancel or reschedule the appointment by 5:00 PM the business day prior to the appointment.
